Hi everyone,

I have been having a lot of trouble finding a civil surgeon in the Plano/Allen or Dallas area that would just transcribe the I-693 vaccination sheet without requiring the whole physical. My husband did his physical within a couple of months (k1 visa) overseas but his vaccination sheet (ds-3025) is incomplete and needs to be transcribed by a civil surgeon.

Please let me know if you know any in this area

I went here in dallas 5303 Ross Ave, Dallas, TX 75206

I waitied several hours from when I first walked into the clinic until I was seen but the Civil Surgeon did transcribe all my vaccinations (I had to have additional ones in the UK after the original medical) and it was $28. They gave me a copy of the i693 and sealed a copy in an envelope with their official stamp etc.
