Medical requirements - serious medical history?

S question regarding the medical - I'm looking at this page as it's probably the hospital I'll be using for my medical here in Japan.

Regarding point number 7, "For applicants with a serious medical history detailed information regarding the treatment is required." I'm a touch concerned. A few years ago, (2010) I had an operation in my lung, and it's left me with a scar on my back. Does this count as "Serious medical history" or can it be safely ignored? Currently (and for the past 3 years), there are no problems whatsoever and the only way I can imagine someone finding out is if I have a chest/lung x-ray, or someone sees the scar down my back. I've made a full and complete recovery to a better state than I was in before!

Should I contact the NHS in the UK and have them send my medical records regarding it, or is it nothing to worry about?

I would definitely get either your medical records or a note from your doctor explaining what the scar is from, how it was treated and whether or not it is a current issue.

If you were interviewing in London, I could tell you that the doctor doing the medical would want info regarding any condition that involved an operation or other medical conditions past or ongoing---high blood pressure, cancer, diabetes, depression, etc. They don't need actual records but a summary from your doctor or the one who knows about your operation stating when, why, treatment, prognosis (full recovery or ongoing problem and meds for life...whatever the case). I don't know the norm in Japan, but you might want to try to get a statement to present. It's because a visa doctor who sees you for 15 minutes may not feel comfortable clearing you without a little more medical info from someone who knows your case.
