First question : I live with my mother and brother, my brother works and my mother has income. Do I have to put them as dependent, along with my spouse on the I-134 form ?

2nd question : Did the form DS-160 replaced the forms DS-156, DS-156 and the 230 ?

3rd question: I'm the petitioner and I recieved the packet 4 and my spouse didn't, do I have to fax her the packet 4 for she can take it to get her medical done ?

No, unless you claim them as dependents on your taxes. Who is counted in your household size:

Yourself

Any spouse

Any minor children, regardless of where they live

Anyone you claim as a dependent on your taxes

The intending immigrant

Yes, if you don't have any dependents, then just a household of 2.

See http://www.uscis.gov/sites/default/files/files/form/i-864p.pdf for the minimum income thresholds for the various household sizes.

Now fiance visa only need ds-160 and ds-156k forms.

And yes you can fax the packet 4.

Or your fiancee can e-mail the dominican consulate one month before of the interview if she hasnt received the packet.

And they will send her by e-mail, she print it and can pay at banco popular and does her medical .
