I have not had to submit my DS 260 yet but I have read people ask about this question.

It's basically asking if you are up-to-date with vaccinations that are required in the U.S. Here is a list:

If you do not have all those vaccinations, it is OK to answer NO to this question. You will still have time to get these done, even during your medical examination.

i have my record here in the philippines . so the answer is yes

the record of your immunization should have a signature of your doctor and the date should also be indicated

to when the vaccine was given. if there is a signature and date then its a yes.
