CFO couple questions

I thought we had everything figured out for fiancee's CFO on Monday but now not sure. Her 4 yr old daughter will attend with her (know she's not required). Fiancee has all necessary documents but we're concerned what documents she needs for her daughter?

Also, I know the counseling fee is 400 php so will they have to each pay since daughter going with her? Finally, I can't seem to find anywhere what the registration fee is for the actual sticker for each of them?

My fiance and two children ages 20 and 11 did their CFO yesterday and paid 400 pesos each and she was interviewed the 20 yo had just a seminar and the 11 yo did nothing other than be there. All had stickers affixed to their passports. They leave Sunday for USA. Documents needed for children are the registration form and their passport and the fee that's it.

its better to call them directly so you will have accurate instruction. thats what i did before i went there. i asked them what requirements should i bring.
