I am looking at the DS-230 (to prepare filling the DS-260) and have a question on question 40a:

An alien who has a communicable disease of public health significance; who has failed to present documentation of having
received vaccinations in accordance with U.S. law; who has or has had a physical or mental disorder that poses or is likely to
pose a threat to the safety or welfare of the alien or others; or who is a drug abuser or addict.
If my wife doesn't have a shot record, should she put "yes"? Or should the answer be "Will provide by interview"? I am confused since this question is asked prior to the beneficiary going to a medical appointment

No the questions apply up until the interview, not after. I assume she will be vaccinated at her medical? Then the answer is no because she intends to be vaccinated and have proof of said vaccination at interview. If she wasn't going to and did not have a waiver either, then the answer would be yes. Your wife would also be denied a visa.

Remember the NVC isn't the one who this form is being submitted to in the end; they're merely checking to make sure all the paperwork is there, t's are crossed and i's are dotted. The form is her visa application that will go to the consular officer in charge of her interview. Write down what will be correct at the time of interview.

btw on the DS-260 i think this is a 3 part question.. so if answered there, you can answer the vaccination as Yes you don't have them yet. There is space to write things in I believe.
