second question of the day,

Every doctor i've called wants to charge for the full medical again, but i do have the vaccine list the ds-3025 or whatever it is, but it's not in a sealed envelope? what damage is this going to do? i'm getting really frustrated with this process.i have the original envelope from my doctor and the cd i can seal that envelope lol im going crazy here, please help.

If you are filing for Adjustment of Status, just include a copy of the DS3025. The sealed envelope you turned in when you entered the country contains your medical results and will be matched up with your AOS petition. As long as you had your medical within the last year, there is no reason to do anything more unless the doctor didn't fill out the DS3025 correctly.

the above post is right. some people have sent just that, but have gotten an rfe. so to be on the safe side, its best to have the form transcribed to the i693.

im in ur situation. I had called the civil surgeons in my area and they needed to do the full medical exam, they mentioned their doctor did not sign for someone else that performed the exam. my husband and I found the immigration clinic and they transcribed it for $40.00, just did it actually days ago.

if u send the transcribed form, call the immigration clinics around ur local immigration field office to see their costs.

I am assuming that you are adjusting from K1. We did not get a CS. We sent in the instructions from the USCIS website. No problem. GC arrived this week. This is a great post on the topic:
