Question about how to put the date on Report Of Marriage

This might be silly but how do I put the date on the Report Of Marriage, it goes like this:

we/I hereby set our hand/s this_____day of_____________at________________.

I assume it should be:

we/I hereby set our hand/s this 12th day of November 2013 at ? .

I dont know what to put at the "at" part ?

Also, do I put the date we prepared it, or our marrige date.

That section is at the bottom where your "solemnly swear under penalty.. " that is the date of you are completing/notarizing the form.

Not sure about the "at" (first instinct is time or place), we did ours at an consulate outreach event, they just had us sign it in the respective spaces, that "date and at" area are blank, the did their stamp over our pictures.

No it shouldn't, this is a report of marriage form for the Philippines.

Thats what I was thinking, I'll just put that, tried getting ahold of the consulate all day, just kept getting the busy signal, reminded me of the good ol days filing the I-129, lol.

Well that makes sense. So the "AT" refers to time of day with the marriage certificate in the Phils.
