Should I include her parents last name? Its been a long time, in Ukraine, since she used this name. I did incude it when applying for fiance visa.

Since you used it in the finance visa application, please include the same in other names used. This gets used in the background check, which is good.

USA

01/08/13 - Approved and GC is order for production on 1/8/14

09/12/13 - Case transferred to CSC. NOA2 received on 09/18/13

08/30/13 - Biometrics Done - No walk ins allowed at this LSC (received on 8/16/13).

08/05/13 - NOA1 (received on 08/10/13)

08/01/13 - Mailed I-751 (received on 8/2/13 - check cashed on 8/5/13)

12/28/11 - Received SSN (applied on 12/20/11, as we didn't get based on DS-230 options)
11/28/11 - Received Green Card (Expires on 10/30/13) - Welcome Letter on 11/17/11
10/30/11 - POE - Houston, TX

Chennai Consulate (40 days)
10/28/11 - Received Visa papers and Passport at VFS
10/25/11 - Interview Cleared Successfully (Spouse was not allowed in)

NVC: (90 days from NOA2 to Consulate)
08/31/11 - Case Completed (Interview 10/25/11) - Received at Chennai on 09/19/11
07/22/11 - NVC Case Number

USCIS: (92 days)

6/21/11 - NOA2 (NOA1 on 3/25/11) - took a month to get to NVC
3/21/11 - I-130 sent to USCIS Lockbox, Chicago.

I could ask "include it in what"? But I believe you are filling out a form that asks for "other names used" YES, list ALL other names used at any time in her life

Im sorry Gary. I can see my question is incomplete. Im filling out I-751, and yes the question is asking for other names used. My wifes name is Elena however her international passport states her name as Olena. So I included both first names, and her last name from previous marriage. So now I was wondering about her maiden name. Do I need to include her first name again or can I just write the last name?

To prevent confusion, I would say write the whole name. A reasonable person may assume the first name stays the same, but if the adjudicator gets confused and decides they need an RFE to get un-confused, you are stuck with an RFE and that delay. I try to avoid those things when I can.

TBH I used only Alla's "real name" and the boys "real names". As you know, Russians/Ukrainians/Belarusians make a plethora of names to call each other. I cannot even keep track of all the names Alla calls her friends. The woman I know as "Natalia 1" is called by Alla "Natashichka" and Natalia 2 is called "Natasha" (or maybe it is the opposite). Alla has been known and is called by some people...Alisa, Alis, Alka, Alichka. Sergey is known as "Serozha", Pavel as ...Pasha, Pashinka, Pavlik...I did not wirte those as they were never "official" and never on any documents. They are informal nick-names. So people say to include them, I did not.

If you included Olena before I would include it this time. Definitely include all last names. But I understand your quandry, if you list all the "first" names and all the last names it can get exhaustive. smile.png
