I've read the pinned thread and some threads about sending copy of DS-3025 and CS transcribes that copy of DS-3025 to I-693, but i'm still confuse, cuz most people still got RFE about the medical even though they have sent the copy of DS-3025 with their AOS packet and made them finally had to see CS to do the I-693.

I'm still confuse what to do, send the copy of DS-3025 or find CS to do the I-693 as soon as i arrive in US so no need for me to worry about the medical thing for the AOS.

1. Which one do you think it's better?

2. Do we have to see the appointed CS or we can go to any doctor to do the I-693?

3. Can we show to the CS the certified letter of vaccines we have from the doctors overseas if they insist to do the medical again (we'll give also the copy of DS-3025)

We did DS-3025 only. That being said, her shots were complete, except for the age-specific ones. If you need booosters or further shots, I think you will need to get a CS involved and have them furnish a SEALED I-693 for USCIS. And yes, take the DS-3025 to the CS when you go. It has to be a CS, not "any" doctor.

The DS-3025 i have stated that it's complete and all vaccines required marked with the "tick sign".

And there's no other marked on the age thing or other thing on the right part (they leave it blank), it has doctor's name and signature as required though.

Actually, i'm sure that my medical is complete, but I'm just worried that USCIS will send me RFE for the medical cuz they lose mine. So i'm trying to figure out what's best to avoid the RFE and save time. Should i just send the copy of DS-3025 or find CS to do the I-693 so i can send it with the AOS packet without worrying too much for the RFE. Maybe i'm thinking too much about the AOS while i'm not even in US yet :P

Btw, i still don't understand what's the difference of CS and the other doctor. Can anybody explain it to me?

You're welcome. Basically, it's a doc who is willing to be "certified " to do the exams for USCIS. They may have to pay a fee for said class & certification, but for sure, they make it up by being in a sort of "exclusive club" that we are forced to use. The same is true for FAA certified flight surgeons. Just a local doc who gets the extra certification, then makes a mint because he's one of only a few docs around who can do the exam. Said exam being THE SAME THING you'd get if you went in for an annual check up. But... they're CERTIFIED!! ;)

It doesn't sound like your DS-3025 is completed properly. There should be something by all the shot lines

Either a shot date or a reason why no shot date should be checked in the right columns.

All the things that are not required of an adult should not be blank. They should have a check mark in the column "not age appropriate". Or "not flu season" for example on the influenza line.

If you want to post a scan of what your form looks like, it would be easier to know what you are describing.

If you have lots of money and don't mind spending several hundred more dollars doing it all over in the US, then you certainly can choose that option. If it is after Oct 1 when you go to a civil surgeon, you will need to add on a flu shot.

I got four kinds of vaccines:

1. MMR,

2. Varicella,

3. Td

4. Influenza

They put all the shots date and marked it.

I'm going to send the AOS packet on October or after which is the flu season and i know i had the shot already cuz they marked and date it.

I attach the copy of DS-3025 i have.

What do you think?

So, I will just send this copy of DS-3025?

And if i still get the RFE of medical then i can ask CS to do the I-693 by showing them this copy and they will not force me to do the exam all over again?

If you get RFE for immunizations, then all you will need is whatever shot(s) are specified by the RFE.

Your medical exam is good for one year. So as long as you submit all of this prior to one year from the exam done in your country, you should be good.

First off the I-693 instructions say [ Section III, 3, c, (2) ]

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

So I think your DS-3025 was not properly completed according to the Technical Instructions for Panel Physicians which tells them exactly how to fill out the DS-3025. Some excerpts :

"If the applicant has completed the vaccination series, the "Completed Series" box for each vaccine must be checked on DS-3025."

"Vaccination history incomplete: Applicant may be eligible for blanket waiver(s).

Completion of a vaccine series is not required to conclude the medical examination, because such a requirement would require multiple visits to a panel physician and could lead to unnecessary delay in the immigration process. If any of the boxes under the "Not Medically Appropriate" heading was checked, the "Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate (as indicated above)" box must also be checked. This box will probably always be checked because some vaccines may not be age appropriate for the applicant."

So going by that I think to be properly completed your sheet should have theses things--

DT, DTP, DTaP- should be marked "not age appropriate". They are baby version

Td- ok because is not older than 10 years, but "completed" column should be marked.

Polio, IPV -should be marked "not age appropriate"

MMR- is two doses. They should have checked the column "insufficient time interval" because it was not medically appropriate to get Dose 2 the same day. It is a four week interval before Dose 2. If it is four weeks past 6-28-13 when/if you see the civil surgeon, then another MMR dose will be due.

Rotovirus to meningococcal- all should be marked "not age appropriate"

Varicella- "completed" column should be marked.

Pneumococcal-should be marked "not age appropriate"

Influenza- "completed" column should be marked.

Results

X vaccine history incomplete

X may be eligible for blanket waiver.

Signed and dated- OK

Because your panel physician marked "completed" and you do have all the required shots listed on the DS-3025, maybe the USCIS would accept with nothing further. But they don't have to because the Insufficient time interval waiver box, for example, is what would excuse you from the second MMR.

You can see why people get RFEs when they think they have all the shots. Sometimes their form is not properly filled out.

You can see if they accept yours and go to a civil surgeon only if you get an RFE. Your shots are done, but the form is not exactly correct and they can get fussy about documentation. And there are so many different people who may get your file. Some may be strict and some may say--this will be fine. Try it. An RFE isn't so bad.
