Hi,

Please help me with a sample format of affidavits for birth certificate. I have a birth certificate but the birth registration date is July 2013. I was born in 1979. I read in VJ that birth certificates which are registered after a year of birth are sometimes unacceptable. So, I am planning to make 2 affidavits stating the contents of my Birth certificate. If someone has a sample of such affidavits, please help me with the format.
