Hello

Right the story first then the question.

Basically i had medical exam in December 2012 at the Knightsbridge Dotors in London. They gave me back to sheets of paper, one was a DS-3025 which was marked as "Incomplete vaccine history" and the other sheet was a letter listing the 3 vaccinations (MMR, Tetanus, Flu) i needed to complete the vaccine history and it was addressed to who ever i chose to do the vaccinations.

I went to my local Doctor and had these 3 vaccinations done.

My question is: i have now got evidence that i had all the riquired vaccinations, but do i have to get these 3 vaccinations that were missing approved by anyone offical like that the Doctor who filled out my DS-3025? what is my next steps when adjusting my status?

Does that mean I go back to the Knightsbridge visa doctors with this form with all my records?

Panel physician= doctor abroad who does visa medicals

Civil surgeon= doctor in the US who does adjustment of status medicals

You need a civil surgeon. Find one here https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV

Read the instructions for the form I-693 carefully http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=eb1f3591ec04d010VgnVCM10000048f3d6a1RCRD&vgnextchannel=db029c7755cb9010VgnVCM10000045f3d6a1RCRD

Prices can vary a lot. Call around.

Ok thanks for the advice, was it ok I got these vaccinations done at my local doctors? Do I just take my previous/new vaccination records to the civil surgeon and they just fill out the I-693 form and that's it done?

Take any records from any place and the civil surgeon will combine them on one official form, the I-693.

So you didn't read the form instructions yet, huh? Read the instructions and it has Step 1-5 on what to do about filling out the form. Also page 3 has info about the form. Don't miss page 4, Frequently Asked Questions, #2.

Things to watch out for when selecting a civil surgeon

  • They can charge you anything they want. $50 is a good deal. $300+ has been paid by some people.
  • They tell people they must do an entire new medical exam.
  • They say you have to pay for a tuberculosis skin test.
  • They just refuse to do the Immunization sheet without a full exam
  • They will fill out the sheet, but won't sign it. Useless.

Shame you two Brits weren't reading the UK forum before your medical. It's often posted to get the shots taken care of usually free at your GP so you are complete on your sheet from Knightsbridge. Then you could skip this civil surgeon step.

It's also a shame the US can't collate all this information properly instead if it being all over the shop. All they say is "attend a medical" there is lots of pointless and hidden procedures that are just created to make the process longer and make more money out of the applicants. You just have to get on with it and do as they say :-)

Anyway thanks for your help

It is all over the place and you have to click many links, but there was a reference in your instructions from the embassy in London.

Special notice for K visa applicants: K visa applicants are not required to meet the vaccination requirements at the time they apply for a visa. However, they are required to meet them at the time they apply for an adjustment of status in the United States. If you take your vaccination records to the medical appointment, it may be possible to use the results of the medical examination in the United States.
