Hi all,

I have been reasearching a lot about filling out the report of marriage form to send to the PI consulate in Washington DC (with all of its supporting evidence), and the part that is stumping me is the requirement to include a self-addressed envelope with "appropriate stamps". The website with the requirements - located here: http://www.philippineembassy-usa.org/philippines-dc/consular-services-dc/faq-dc/#marriage never says how much postage to include. For those that have filed this report through mail, how much postage do I include on that envelope? (Keep in mind I'm not having a passport name amended - just filing the marriage report. So no passport will have to be mailed back to me).

Also, the only address I could find on that site for the PI consulate in DC is:

For those who filed in DC, is this where you sent your forms?

One last question: On the form itself in the section pertaining to the husband it asks for NAME (last name, first name, mother's maiden surname) This only makes sense for the wife's section, but not for the husband. Did any of you use the mother's maiden name in the husband section? I've never had my mother's maiden name as part of my name so it doesn't make any sense to document me that way. Middle name would make much more sense. What did you all put there?

For the stamps - just the one standard forever stamps or worth $.45, they will just mail you a copy of the approved registration.

For the address... remember that you are going to report the marriage to the consulate where your marriage took place... not where you live, for instance for me, we got married in Hawaii and we live in Pennsylvania, I reported the marriage to the consulate of Hawaii. If the place where you got married falls under the jurisdiction of Washington, DC, then the address you indicated is the right address.

Mother's maiden name is your mother's last name when she is not married yet to your father... same with mother's maiden name of your husband... your husband's mother's family name, when she is not married yet.

You are welcome... the name thing is the Philippine way... a lot of people get confuse about it...

I would also recommend to go to the Philippines Embassy website and sign up for the email alerts. They do outreach all the time. When it comes time to change the name on her passport or register children it's great.

We are in Kentucky also, and we did our report of marriage and passport name change in Charleston, WV when they did an outreach there.

When I sent mine in last April I forgot to send the check. The lady at the Embassy actually called me on my Cell Phone. Told me to send the check to ehr attetion and she would hold it on her desk until she got it. How is that for service !

We went back to the Philippines in Sept and had our formal ceremonial l weeding. They already had a copy of our marriage registered in Cebu , so it happened quickly.

I usual weed in the spring... :rofl: (sorry man, had to do it! :rofl: )

The weeding is part of the custom with her people in the Cebu province. It involves taking the weeds from flowers to signify a perfect Union.. We also had a wedding.

If we did weeding here in the desert there would be nothing growing as weeds are the only thing that is tough enough to survive in our yard. :lol:

LOL Of course I was BS'ing about the weeding ceremony.

I spent a few months in the high desert near Barstow. It was nice for some odd reason. Very cool in the winter but in the summer time, it was unbearable.

No, the weeding is the test drives you take before selecting the model you are taking home.

We had the same question on passports along with other questions and just gave up and waited for the consular outreach to come here. We did report of wedding, births, and passport all at the same time. Plus it was nice seeing all those hot little Filipinas in the same place at one time.
