On the DS-156K question #5 it asks for my fiance to list the name of of unmarried children under 21, and to send a copy of child(ren) birth certificate. Well we have a one month old. I am waiting for him to get here to submit her birth certificate, because since we are not married he can not be put on the birth certificate, unless he is physically there to acknowledge paternity. I have a verfication of birth for the time being. Do you all think I should send of a copy of this instead, or should I just not list our baby's name on the form? I did include in application that we were expecting a baby in Oct when I originally submitted our application.

I copied our daughter's birth certificate,the affidavit of parentage(had it notarized in the DR), and the application to have him added to the birth certificate. If you can not get the affidavit of parentage as it has to be signed by both and notarized. You can just include her name and then at the interview (if you will be there) explain to them about not being able to add his name.

I would list the baby on the form and provide what evidence you can provide for now. I would not omit the child since you do not want to give Lagos any reasons at all to deny.
