Please look my details first and try to answer my question listed in last of page .

I came in USA last 9 months and change 2 addresses. (From Jan 2012 to Present)

US Address 1

US Address 2

Before I live in USA I lived 1.5 year in Germany and Changed 2 addresses: (From Aug 2010 to Dec 2011)

Germany Address 1

Germany Address 2

Before I lived in Germany I was living in my home country.( From birth to Aug 2010)

Home Country Address 1

Home Country Address 2

Now please help me to write below 2 question.

Q: Applicant's residence last five years. List present address first.

Do I need to write Germany and Home country Address here ? OR only 10 month US address.

Q: Applicants last Address outside the USA of more than 1 year??

Which Address here?

Just answer the question the way it has been asked. Last 5 years. starting with USA's two address, Germany and than your home country. Most recents first.

for second questions, I would say Germany as you lived for 1.5 years.

There should be a contact number, should you have any questions. also confirm with them.

List all your addresses from the past five years.

Then, list your most recent address abroad where you lived for more than a year.

I know it can seem confusing because you are repeating some of the same information, but it is because the form is also used by people who have lived in the US for the past five years so in their case the second question would elicit new information :-)
