can we still have the medical appointment done w/o appointment letter ?

It is no problem. It is a walk ing and no appointments necessary. However they want to see your appointment email for your interview or an offical document from the embassy that has your Manila Case number. Good thing my fiancee printed out her September 7 visa appointment email.
