Last Name

Hello Everyone,

Had a questions with regards to which Last & Maiden Names to be used.

My wife would like to take my last name so for all the forms (485, 325A, 864, 765, 131 & 1145) is it ok to use my last name for hers?

Also for the maiden name - can she use my First Name or does it have to be her before marriage last name?

Yes she can use her new last name on the forms.

A maiden name is by definition the last name of the woman before marriage. It is not by definition her husband's first name. I am not sure why you are asking that.

Harpa is right, her maiden name was her last name when she was still single, have nothing to do with you. And Yes if she want to get all her documentation with her new name, fill all the forms with that name

I'm sorry, what I meant to ask was Middle Name not maiden name.

Is there a recommendation for Middle Name (husband's first name or maiden last name)?

Usually people keep their own middle name the way it is. If you want a middle name change, I believe you will have to get it approved through the court system for a legal name change. The marriage cert is enough for a last name change, but not for a middle name change. Some states will allow a person to use their maiden name as a new middle name, but none that I know of would allow the middle name to be changed to the husband's first name without a legal procedure.
