I had my medical examination done in Germany in June 06 during my K-1 Visa application process.

So I am one of the people who only need the I-693 A Vaccination Supplement Form filled in for the AOS application.

This is where my trouble starts, I can't find a civil surgeon who is willing to do it.

One told me point blank, I do it his way or no way and if I keep making trouble (yea right!), they might not do it at all.

Now my husband called a few more doctors and we found one who said they would fill out the form if I bring proof, that I had the exam done already.

In Germany the doctor never gave me a copy of the D-3025, I only have my vaccination record in a little book and I still have my chest x-ray.

I called my parents to get in touch with the doc in Germany to see if they still have a record of me being there and I hope and pray, that they can still send me something.

If not...any ideas what I can and should do now?

In my opinion the doc could care less if I had the full medical exam done already, I mean, I don't want him to sign anything he didn't do as far as tests go, I just want him to transfer my vaccination record on that form. If that is not correct or enough, I guess the INS will get back with me, not him...

Now I was wondering...since it is not the exam, could I go with that Supplement form to another doctor, ie. one on the AF base where my husband is stationed at?

Any ideas, suggestions,...on what I can do now?? :help:

We had my husband's sister get that vaccination form from the doctor in Morocco. She sent it to us and we were able to find a civil surgeon who for 10 dollars would transfer the into onto the I-693. We had to drive about an hour but just keep calling around. Make sure you tell them you don't need the medical because you just had it in June.

I don't know at which doctor in Germany you had your medical, but I do know that the one in Frankfurt and in Hamburg is very helpful when it comes to send or fax a copy of the DS- 3025 to you.

If they refuse it, tell them that on the bottom of the form is a reminder" Give copy to applicant".

If they still refuse to give you that copy, you need to find a civil surgeon from the USCIS site..... and hopefully find one, who will transfer your records from your "Impfpass" to the I-693 form. It makes it easier, if your "Impfpass" is an international one.

Even with my DS- 3025 it took us loooooooooooots of phonecalls to find a CS, who was willing to just transfer the information without giving me a whole new medical.

Thank you. I have my dad call there tomorrow (it was the one in Frankfurt)and I hope, I can get it faxed or send here.

Hi girls,

Check the forum with all the Germans (VisaJourney.com > General Discussion Area > Regional Discussion > Europe & Eurasia (except the UK and Russia)), they can certainly help you! I didn't come with K1 so I don't know anything about the German docs in Frankfurt, but most of the other girls went through Frankfurt. They will help you.
