"Have you ever applied for permanent resident status in the US?"

I have! When I was single my mom applied for me but I only have the I-797C notice of action (Approval notice). Should I put the receipt date or the priority date? We did it with a lawyer so I don't know (and he doesn't call me back!). Also, for the place filing should I put the place my case was transferred for? Should I send a copy of my I-797C, Notice of action?

Thank you!

Send a copy of the notice, use the receipt date not the PD.
