In the VJ "Assembling the I-485 Package: Checklist" number asks for "Copy of the non US Citizen Spouses Passport (biographical page as well as entry stamps).".

Do they need copies of every stamp or just the one when she entered the USA ?

"Part 1 Family name"; do I put her maiden or her new married last name?

In the VJ "Assembling the I-485 Package: Checklist" number asks for "Copy of the non US Citizen Spouses Passport (biographical page as well as entry stamps).".

Do they need copies of every stamp or just the one when she entered the USA ?

"Part 1 Family name"; do I put her maiden or her new married last name?

They want a copy of the bio page, her stamp from entering the US, a copy of her K-1 visa and a copy of her I-94, front and back.

If she took your last name when you got married, then use her new married on the forms. The name you put on the forms is the name her EAD/AP and green card will be issued with.
