It may be difficult to replace something that the OP was never issued.

But if the IO took the I-94 from her, then perhaps USCIS

can find some record of it in their system?

Ok after checking online from Dublin to see what I had to hand in I believe all I had to show to Immigration in Dublin was my passport with B1 visa and CBP Declaration form http://www.cbp.gov/linkhandler/cgov/travel/id_visa/i-94_instructions/sample_cbp_decla.ctt/sample_cbp_decla.pdf

The I-94 is handed out onboard when arriving to the US for travellers who need to pass immigration in the US. I had already passed immigration in Dublin so I was not been inspected in the US so was not given this.

In another link from the Dublin embassy I found that the CBP places a small white card (I-94) in the passport. This never happened either.

Sorry all I was getting confused at first with the I-94 and the CBP declaration I filled out at Dublin airport but I know nobody gave me a I-94.

Sorry for all the confusion people.
