I-693A vaccination supplement

I'm always anxious to ask questions that may sound stupid but I honestly did search for it.

Now, this I-693A vaccination supplement - was he supposed to get that in the UK? Can he write to the London doctor & get it or will he have to pay? He's still in England, so if you could tell me the best way to get this, I'd appreciate it.

He said he sent his vaccination records with his medical form so I'm guessing he has nothing now. Shuold he get a new set from his GP?

I think I have everythign else we need to file for AOS except this.

Dev - they should have given him the DS-3025.

The I-693A is a USCIS form and the consulate physicians don't have it available to them.

Vaccination records (DS-3025) are sent to the embassy with the emtire medical exam, but he can request a copy.

You will then have to present this copy to a Civil Surgen here in the US, to transfer the info to the vaccination supplement (I-693A).

Before I left Morocco, I went back to the consulate physician and asked for a copy and got it same day.
