Hi to you all, I've been calling various places to ask about the form I-693 to be filled out by a civil surgeon. No one understands what I'm talking about, what exactly should I ask so they know what I need. Thanks again for all the help.

Have you tried " immigration vaccination sheet to be filled out and signed by a doctor " when you call? Thats what worked for me when I was trying to call for information on prices.

Good luck on this. This was the bane of our existence. Even "civil surgeons" will have no idea what you are talking about, let alone their receptionists.

I've been calling all around and they all insist on needing another physical done. I'm so frustrated...I guess we need to spend more and more money. Did any of you get and RFE when sending only the vaccination supplement sheet?

If you are applying within 12 months of your K1 medical being done AND if your vaccinations are completed per your DS-3025, you can send DS-3025 (vaccination sheet) and do not need another medical done.

US citizen since April 2016

ROC completed April 2014

I've been calling all around and they all insist on needing another physical done. I'm so frustrated...I guess we need to spend more and more money. Did any of you get and RFE when sending only the vaccination supplement sheet?

Yes, people send in only the vaccination sheet they get from their medical(DS-3025) and don't get an RFE. Theirs are completed though. Unfortunately, since you went through Juarez, you do not have a completed DS-3025 vaccination sheet to send in instead of seeing a civil surgeon. You would definitely get an RFE because of that. I know it is frustrating. Some people just go ahead and do the medical just to get the I-693 and vaccinations they need done. You can try telling them the I-485(page4, #8) and I-693(page 4 #2 C.) instructions both state you do not need another medical if your medical was done within the last year. Tell them you can print out the instructions and bring them with you to prove it. You might need to try and call civil surgeons that are a little farther away from where you live. We found one willing to do my husband's without another medical that was a 2 hour drive away.

I know, I am so taking the whole I-693 so they could see we don't need another physical. We are driving two hours to see a couple of civil surgeons, by phone they all say the medical is needed. So I hope by going there and showing them what I'm talking about they will comprehend. Thanks again :)
