1040 no last name, what to put?

So filing our first joint return, including form w7 for her ITIN but the issue here is that she has no last name.

So had anyone found the right answer to this?

Do I leave it blank on the 1040 and the w7, or do I wrote N/A, or what?

If I follow the instructions literally then I would leave her last name blank, although something tells me that this will cause confusion.

one of my bestfriends only have 1 name with no family ..just wrote N/A :)

* March 2nd 2012 --> I-130 sent

* March 5th 2012 --> I-130 Delivered at Chicago Lockbox

* March 7th 2012 --> Got NOA1 by SMS (Vermont Service Center)

* March 14th 2012 --> Got Hardcopy of I-797C in the airmail

* June 29th 2012 --> NOA2 (114days)

* July 5th 2012 --> Got Hardcopy of NOA2

NVC

* July 2nd 2012 --> NVC Received

* July 24th 2012 --> NVC Case Number and IIN received by e-mail

* July 24th 2012 --> E-mailed the DS 3032

* July 24th 2012 --> AOS Bill Paid

* July 26th 2012 --> AOS Bill Shows PAID

* July 28th 2012 --> AOS Package Sent

* August 2nd 2012 --> DS-3032 accepted

* August 3rd 2012 --> IV bill invoiced & PAID

* August 7th 2012 --> IV Bill Shows PAID

* August 17th 2012 --> IV Package Sent

* August 24th 2012 --> NVC Case Complete

* September 15th 2012 --> Interview Date Assigned

US EMBASSY JAKARTA

* October 16th 2012 --> INTERVIEW (Approved)

* October 19th 2012 --> Visa in Hand

Then she has no first name either. List her name as shown in her on her SSN (or ITIN) card. If only one name, use the last name field. The system locates files based on the first four letters of the last name.

No SSN card yet and ITIN card is applied for along with the return being sent in so no ITIN yet either.

Timeline

i don't have a last name either, so i just write N/A, no problem with that and being approved without RFE. on the mails from consulate, they put my last name as fnu (family name unknown). SO N/A it is :)

So just an update, I called the IRS and asked. They werent sure either so they made an appointment to call me back the next business day. When they called me back, right on time I might add, they said specifically "leave it blank, and if the IRS needs further information from you they will contact you".

Hope that works, and you kept the name and id number of the person that punted that ball down the field and gave you that instruction. The IRS information line has a worse reputation than the UCSIS information line.

Yes this is my concern, didnt save his ID, if it was a problem then nobody would care about him anyway.

I wouldn't worry too much. Usually if they have enough information to process the return, they go ahead and do it. If the math doesn't work, or you misapplied a deduction or exemption, they send you a correction along with a bill, or a reduced refund.
