What is the right form to use for renewing expired Philippine passport using married name?

I already submit the report of marriage.

You have to go in person to the consulate that serves your area of the country. They do electronic passports now, which is they take your fingerprints and picture on the computer at the consulate.

I used the amendment form. I just need to gather the renewal of passport requirements plus the report of marriage duly accomplished form.

Thanks.. i read an update saying that if passport is expired before applying for amendment you have to renew passport using the passport application form ( as if you're

applying for the first time but check the renewal of passport box in question 9a). which i don't see that 9a..

so confused with this forms...
