Vaccination Supplement Questions

Hi Everyone,

I've recently gotten my K3 visa approved, and will be filing for AOS later this month. I had a medical exam done for the K3 and from reading, it seems like I won't need to do the full medical which is good for one year. I didn't do the vaccination portion, as it was not required. My question is, do I download the supplement and have the Doctor complete it to forward with the I485 or do I wait until the form is generated and sent to me to be completed?

Hi ,

Yes you do need the Vaccination Supplement completed. Which you need to take to the AOS Interview. I went had mine checked today. Had to have another three vaccination which where not done in the UK. In total this came to $202.00. I hope this helps you.

The form will not be sent to you. The Civil Surgeons should have the form.

Thanks Yodrak. I contacted the physician that did my examination for the K3 Visa, and he has no problems with completing the DS3025 for me. Is this form acceptable to send with the I485, or should I just let him complete the I693A? Is he still considered civil surgeon outside the US?

Hi, be sure to call a couple of civil surgeon. They all charge different amounts of Dollars.
