I have 2 questions while I am filling out my form.

Question 14: National Identification number? What do I put there? As a SOuth African we have a Indemnity Book with our ID number in it, is that what I should put there?

Question36: Anyone filed Immigration Visa on my behalf? Do I put my fiances name there or leave that one blank and say no?
