what is this form needed for?? do i have to fill it out for my fiance since he is a male???

Your fiance should really be filling out his own forms that he needs to take to his interview. He will need to understand what it is he is signing his name to, as they make him sign the forms in front of them there. The DS-157 is stated to be a required form for males, but CDJ seems to not care about it. My husband filled one out and took it with him, but they did not want it. However, I would have your fiance fill it out and take it with him to his interview just in case. Better to have it if they want it than be without it.
