Hello Everyone,

My fiance and I are working on her checklist:

http://photos.state.gov/libraries/poland/275705/cons_iv/IV-F04%20Instructions%20for%20applicants_rev_%20Dec%202_%202011.pdf

We have some questions:

For Question 3:

My Fiance has never been married or divorced before does she still check it off or leave it blank?

For Question: 5: She has never been committed of a crime or gone to court etc as stated in her police records would we also still check question 5 off or leave it blank?

Last but not least:

For Question 7:

She currently has a part/full time job but she wouldn't be able to do that when in the US to she gets her social security number and authorization to work. That being said I am going to support her and I have already filled out the I864 does she also need to do one of these forms. Or if she brings evidence of my support (pay stubs, taxes, and the I864 that I am filling out is that sufficient?).

Again all in all would we simply check off the boxes even if that information does not pertain to us, we don't want the embassy to think we forgot to get the required information.

Thanks in advance for your help?

We know that, that is not what our question is as this is different for everyone based on their situations.

All my main question is should we check off things that do not apply or leave them open.

Then the other questions as stated above, which does not typically apply to the sample forms.

For the checklist, don't check off anything you don't require. It's just a guideline as to what needs to be brought to the interview. If it makes you feel better, just write "N/A" next to anything that does not apply to you.

You are the petitioner, they care about your finances, not the beneficiary's. She should be providing your signed I-134 (the I-864 isn't usually required until AOS) and your supporting evidence of tax returns, pay stubs and letter from employer.

Thank you Coconuts that was very helpful :)

Also for the police record that would need to be translated to English as per my understanding that all foreign documents must be translated to English yes?

Sorry for all of the questions but its always better to be safe than sorry :)

On page two it states "All documents not in English, or in the official language of the country in which application for a visa is being made, must be translated into English by a certified translator"

If you find out differently let me know but I don't think you need to translate it in english.

No translation is needed (for birth certificate) if the document is in Polish and your fiancee is Polish. Court documents, such as a divorce decree would need a translation. Police report does not need translate.

That said, you will need a translation of the birth certificate into English when you do the AOS (as well as other court documents

Correct on the I-134. On this particular form, the Polish version of the instructions requires it to be notarized, while the English version does not. We did, did not want additional delays. Another Polish couple that interview about same time also notarized, however a couple from Ukraine that also interviewed in Warszawa about same time reported that they did not notarize and was not a problem. Maybe the requirement is only for Polish people or maybe the instructions need to be reviewed; our goal was no delays or excuses so we did it.

Leaving blank is ok. This form is only used for the Embassy to send you the next instructions about medical exams and such, which truly you don't have to wait for; they are all published in the Embassy site and for example, you can move on your medical exam and other tests they want. Obviously, you want to have all documents that pertain to your case ready. Interviews are scheduled towards end of the month for the entire following month, that is, around Jan 24-25 is when they will start scheduling and only then will they notify you. They also publish the schedule in the beginning of the month, you Feb 1st, you can see it in case a notice did not reach you (it's by case number)
