Also, I would hesitate to have my wife put something like this in there if we had been intimate prior to marriage. This wont look very "decent" because Pakistan is an Islamic country. Even though these things happen I was told by others that when asked about premarital relations they did not answer truthfully due to the laws in Pakistan against premarital sexual relations. I do not think it is a good idea to mention it at this point. I also doubt it will help the case.

Agreed. Pakistan is already considered a high fraud consulate. Pregnancy before marriage is considered taboo and against cultural norms, raising red flags. Best to save it for the interview, where it obviously will come up and you will have to deal with it.

I agree with folks that you shouldn't supply it, but don't lie if asked. And I think the chances of him being there for the birth are slim to none, because of what seems to be mandatory AP of all men in the MENA region :( You can check out the MENA forum and the AP/waiver forum for more details.

Timeline

I do not know about USCIS but the Consular Officers at the Embassy are trained to understand the local culture and what is acceptable/unacceptable in that culture. If I were a Consular Officer determining your case (and since it is a fiance' visa), I would keep in mind the cultural background of your fiance and suspect possible immigration fraud that perhaps he is trying to force his way through to the US since you are pregnant. I do not know the unseen but a Pakistani male who is being sponsored by his pregnant USC fiancee can raise some major flags and doubts towards the authenticity of this union. I'm in no position to judge/analyze your situation but whatever facts you've mentioned about your case, my advice would be to not mention the pregnancy and if you truly want him to be there with you as soon as possible, I think you should go marry him and file for a CR-1 asap instead of taking a risk with a fiance visa. As far as the duration of the process goes, I know cases where the wife filed to expedite the case on health/humanitarian grounds and that sped up the process but I do not know of any cases from Pakistan where a pregnant fiancee had approached USCIS to speed up the case/background checks.
