HELP!!! FORM I-134

Hi everyone!! ill trying to get papers done mean while i wait for my NOA2 for my fiance who is in mexico.

On form I-134, question 3 where it states "This affidavit is executed on behalf of the following person."

Would i put my fiances information or my information?? and where it states "relationship to sponsor"... if its my fiances information would it say father in law or something else if my dad is doing it even though were not married or would it be something else???

then on the same question it say says spouse and children accompanying or following to join person??

would this be my name go on this part??

im really confused pleae help thank u !!

2. "Relationship to sponsor" who are you completing the I-134 for?

3. "Spouse and children accompanying or following to join person??" The names of any children or a spouse goes there.

for # 3 would i put my name under spouse even though were not married yet. im doing the fiance visa for him
