Can somebody help? i am from philippines and im doing documents for AOS. they say i need the form I-693. my result on here is VACCINE HISTORY INCOMPLETE and below that it also said APPLICANT MAY BE ELIGIBLE FOR BLANKET WAIVER(S) BECAUSE VACCINATION(S) NOT MEDICALLY APPROPRIATE AS INDICATED ABOVE.does it mean that i will not be getting another shots for vaccine? i had my medical last june 22 i know it last for a year but i still wanna know if something has chnage and how much should i pay the dr? thank u!

Your MEDICAL REPORT will expires after six (6) months not one(1) year..

Good day to you,have you read the instruction of the Form I-693?it says if the medical taken within a year overseas the applicant DONT need to Redo or repeat the entire medical,so it means that it is valid for (a year not 6 months).if the vaccination is incomplete then you need to find a civil surgeon to transcribe the vccination part.

Fill out the form I-693:just part 1.info about yourself.part 2(only the vaccination part c/0 cs.part 5:the cs info and certification....

If your vaccinations is Incomplete go to uscis.gov and find civil surgeon...if your medical is taken within a year then no need to have another medical,bring your medical overseas,your passport,nd show to the sc your medical paper work from Pi..mine they gve me 3more shots..and we pay 235 dollars.somewhere in foothill ca..

thanks u so much ronluvme..now i know that i will not pay lots of money for this form since the CS will just transcribe whatever on my copy.

If you are ready to file AOS now, then you do not have to have another series of shots of vaccine. Some people just sent their copy that St. Lukes gave them and they got approved.... Some people received RFE for the Form I-693...

If you do not want a possible RFE, find a civil surgeon near your place and have him transcribed your medical record in the Form I-693, some doctors do it for say less than $50, some doctors insists on doing another medical and it cost around $190-$300. When you go to the civil surgeon, please insist that you already have your shots.
