The US embassy is not a medical center.

Are you very sure that P4 is not telling you your interview date is on November 3rd?

Generally the medical is yours and yours alone to deal with.

The embassy sends packet 4 which contains the interview letter and appointment time.

Your packet 3 should've have this information or you can check on the embassy website, they usually have a doctor address there. It is not just any doctor, but doctor authorized by embassy.
