Im a bit late sending out this application and im in a bit of a bind...Can anyone explain to my what this i 693 form is about? I filed i-485 form with my fiancee and she already got her vaccinations and medical done for the interview at the consulate she acquired the visa from? Why do i need to do this again? and if so, can i do it at a later date and just send the other packet in?

2. i still havent filled out a SSN Application yet and i wanted to know if i need to put her full name given to her at birth or can i put my last name on the i-485 form(1st line)?..

1, the 693 is for if the vaccinations are not complete.

2, You put the name that you want on the green card. So if shes using married name then you put married name.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

hi,

1.take a look if the vaccinations from overseas where she had the medical isnt complete,if not you need to make an appointment to transcriber her vaccines.be very aware some civil surgeon may insist to redo the whole medical(physical exam)bring the vaccination that she had and explain to them. the 1-693 forms consist of 6 or 7 pages part one is information about her;the part2.is the vaccination thats the only part that need to supply by sc and the part 5 the certification and signature of sv.nothing else.,go to uscis website and find the accredited civil surgeon base on the list..

2.she can use her marraied name depending on the marriage certificate,u have to include certified copy of your marriage certificate to file the AOS showing her name changed,
