There are no exit visas from Ukraine. Your answer is nonsense. Please refrain

This is a specific Ukraine question regarding leaving UKRAINE with children under age 16. So far two people who have experienced leaving Ukraine with children have answered. The others are guessing. Please stop guessing.

The same letter CAN be used, we went to extreme lengths to make sure ours specifically covered that but I am an extreme kind of guy when it comes to such things because I do not allow stupid people to interfere with my life and that of my family and do all I can to protect myself from being interfered with by stupid people.

If the letter she had in 2009 was for permanent relocation to the USA (which it should have said in order for her to get the K-2 visa) then you are fine. Just show the same letter.

adoption is a state matter, if she is a single parent and they are both legally in the US and she agrees to the adoption, it should not be difficult.

Do any of the other surrounding nations still require one? I realise its a bit of a soviet relic I suppose.
