Your fiance needs to go with the letter because they will not complete the medical without it. When the medical is complete they will tell your fiancee to come back in 8 days to pick up the results.

While you wait for the results you should use that time to pay Sogebank $350us for the K-1 visa

When you go for the medical it depends on the age of the person. The older the person the more money you pay for the medical. If he is about 23-49 is will cost $300us. If he is older than 50 will pay $450us.

Next your fiancee needs to go online to fill the form called DS 156 and print out 2 copies. After you get the Medical results he needs to take all his papers to Haiti messenger. Make sure he has all the documents he needs from you. The most important are Affidavit of support they have to be ORIGINALS not copies. You can send by DHL, FEDEX< or UPS. If you have your interview date already you just wait for the day and go to the consula for your interview.
