Questions about filling in DS-2001 and Package 3 Supplement Sheet

Hi, dear all,

I have some questions needed to confirm.

1: When fill in package 3 supplement sheet, should I only answer the several questions in Chinese (except name field also need English), do I also need to answer them in English?

2: We should fill in DS-2001 form and send it to NVC with DS-230 package, right? Should I fill in DS-2001 form in English or Chinese, especially the Applicant's Address field for receiving interview letter? What the date means in the form, signed date or plan to interview date?

1. I had my wife sign both but I think the intent is to let the beneficary sign in either English or Chinese, whatever is more comfortable.

2. Not familiar with the DS-2001 but any address that you plan to use to have something sent in China is much better in Characters (Chinese). I would think it is signed date since you don't have an interview date yet correct?

I don't know if I need to fill DS-2001 or not, I just did some study and see you have to fill a form to tell NVC you have prepare all the documents needed and ready to interview, and send it to NVC with your DS-230, so what's the name of this form(DS-2001 or OF-169)? should it be filled in beneficiary's home language or English?

The OF-169 is what my wife signed. It was one booklet with the first part in English and then the second part (duplicate) in Chinese. I think you can sign either. I think the purpose is to eliminate any many blue slips as possible making it clear what you are going to need for the interview. I read the English part, my wife read the Chinese. :yes:

I think signing in Chinese is fine. It is less a legal document that the Consulate needs and more fair warning so you can not complain they did not inform you certain documents would be needed.

1. Chinese, except for the casefile #.

2. Disregard - that's not used at GUZ IV for an IR-5, at all. The OF-169 IS USED, but not if there is NVC Processing.

3. You still doing NVC EP? All the forms are sent in via email, everything is scanned.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !
Ya know, you can find the answer to your question with the advanced search tool, when using a PC? Ditch the handphone, come back later on a PC, and try again.
