I'm trying to gather the documents so I can arrange the interview in London. On the London Embassy page ( http://london.usembassy.gov/iv_15.html ) I press on the DS-156 form link on the right side of the page which then takes me to a page with an error message - cannot connect to the server. I tried to load it in a different web browser incase it didn't like the one I use but still can't get the form to load. Does anyone else have the same problem? I did find the form on another embassy website and saw it has a barcode section - does the London one generate a barcode or is it ok to use a copy of the form from elsewhere? Kinda stuck on what to do...

Ok thanks, I tried updating the Reader and got the same problem, maybe some setting on my router/firewall. My fiance just managed to load the form so it appears its just me with the problem. I got to go elsewhere to print anyway since I don't own a printer, so hopefully it will work there :)

Maybe your fiance could fill the form in for you, submit the form to get the bar code and then print to PDF and email to you to print? Just a thought...

Hello, I am getting a similar problem with this DS-156 form. I've updated my adobe reader and tried a different browser (IE instead of Firefox) but when I complete the form and hit continue it opens Adobe and then the following message appears: Adobe Reader could not open DS156_Complete.pdf because it is either not a supported file type or because the file has been damaged (for example it was sent as an email attachment and wasn't correctly decoded).

I don't know what to do other than ask my fiance to try and fill it in. I think I will call the US embassy information line and pay an extortionate amount of money to not get anywhere...

This is so frustrating as I've done all the other forms and have my medical exam in 1 day! :(

Anyone have any advice?

I had the same problem and the one that my other half got in his packet was oooollllldddddd. But, of course, if that is what they send him, then they must be okay with him using it, right?
