police certificate is already in hand, we have tons of photos already in a binder for the interview and after talking to the drs, as a K1 visa applicant i do not need my vaccinations taken care of before i reach the usa. so my plan was to have them done by my GP on the nhs before i fly out.

any of the forms i should print out and fill out before they are sent by the consulate as we are still waiting to hear from London?

Hi there

Probably best to check out this link:

http://london.usembassy.gov/iv_15.html

This lists clearly the documentation you need to collate, on top of the forms that must be returned to the Embassy. You will need to complete the forms Jock mentioned, but also DS157 (2 copies).

Whilst you're with your fiancee, it would be worth asking her to gather information for the Affadavit of Support I-134. This is the main document that she'll need to complete. Other documentation will need to be completed by you.

Hope that makes sense.

Yeah, the I-134 is the only thing your fiancee will have to complete. So if you can take that back to the UK with you it will save on some postage costs :-)

Rugby--get some passport photos taken in the US because you will need 2 US size for the embassy and one (US or UK) for the medical exam. All you need to take back from your fiance is her signed I-134 and two proofs of her income (tax return/W2 or tax transcript or employer letter or pay slips). Make photocopies while you're there so that part is done. You do not need photos together or proofs of your relationship for London. Everything you need in London is all about you except for her financial support.

Are you landing in London and going straight to the medical? Take passport, police certificate, passport photo and this medical questionnaire filled out. It's on page 9 of 9 http://photos.state.gov/libraries/unitedkingdom/164203/cons-visa/iv_medical_letter.pdf

Forgot to say this:

While you can get a visa without any shots, if you get everything recorded by Knightsbridge before it goes to the embassy, you won't have to pay a civil surgeon to put those shots on a new form (I-693)in the US. Your COMPLETE form DS-3025 (shots) will be accepted by USCIS. If you go to your GP after your medical shot record is done at Knightsbridge then you have extra hassle and $$$ in the US before you can file for AOS. Tell Knightsbridge to hold your form until you get the shots and have your GP fax the details. If you haven't had chickenpox, then you can't get complete at Knightsbridge so doesn't matter and you'll have to pay a civil surgeon in the US. They do take your word for it. If you haven't had chickenpox you'll need a varicella shot in the US (not generally available in the UK).
