K1 have difficulty getting expedited for anything but military deployment, but you could always try. Get letters from his doctors stating the nature of the illness, the amputation, that he will need help after the surgery and there are no US family members who can help, then use that to ask for an expedite.

If you get there after the amputation, it will not affect the visa as such, other than possibly the Affidavit of Support, if he has to leave his job/ cannot work after the surgery, especially as some US embassies, especially Manila, do not like co-sponsors.
