Should i have I-693?

I just came in US on K1 a week ago and gonna file AOS soon. I had medical check up on March 2011 in my home country for Embassy need before. You know that the Embassy doesnt give us the Medical Record that we have at that time, what i have now just the proof that i have some vaccinations. So, should i have I-693 form for filling AOS and have a new medical check up report?

What exactly is that proof and what do you mean by some vaccinations? If you don't have form DS-3025 and it is not ticked completed, then you have to get the missing vaccinations and have the records transcribed to I-693. You don't need a new medical check up report, just parts 1, 2 and 5 of I-693.

Thanks for the response. I had that DS 3025 but that was submitted to embassy before. What i have now just a paper from Medical clinic in my home country that i have some vaccinations like polio etc.
