Smoking is allowed! Being a heavy smoker will not affect you getting the visa. Should you have developed cancer from it, they may ask for proof of medical insurance before granting the visa.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

Anything health/ medical related is very expensive in the USA, so health insurance is a very good idea, but not usually required to get a visa unless the beneficiary is old or has health problems.
