You have to fill out the DS 156 online. I suggest use Mozilla Firefox or IE.

I had bad experience in using google chrome.

yes sis yan ung link na click ko but ung link nalumalbas is Server not found,, and pinagw ako n din sa friend ko pinptry ko kung llbas ung form pero ndi din daw saknya ,,thnks nwey sa reply

hi mga sis, we both have the same prob...ano bang nangyari sa DS156 ngaun? yan na lang din kulang ko..nakuha mo na ba confirmation sa NVC sis? kelan ka na approved? ours was approved last May 12, 2011 (NOA2)..nagpa appointment kana sa USEM.

i have the same problem but decided to just make another ones ( for me and my 3 year old son-- the first ones had some unanswered questions so the lawyer asked me to make another)when i already got the letter from the embassy which im still anxiously waiting until now.. approved last april 12,nvc letter recieved april 27 tagal!

yeah i tried it too.. maybe bcoz they're updating the DS-156 form, it will gonna expire na kc within this month.. i tried to open kc the 2 other forms and na-open namn... uhmmm sna maglabas na sila ng revised form ng I-134 at ska DS-156

hi sis magpappappointmnt n po ako bks yup nakuah ko n po ung eligible letter from us embassy manila kya pwede n ko mg pa medical by tuesday
