Hi,

I was looking over the DS 230 form which I am eventually going to have to fill out and have a couple of questions I am hoping someone could help me out with.

Question 14 "Spouses name"

Do I leave it blank or put NONE since we are not married yet?

Question 30 says "List all places you have lived for at least six months since reaching the age of 16"

Does it matter if there are gaps in time? I have moved a lot since I was 16 and some places I lived for less than 6 months. Should I include them anyway?

Question 35 says "List dates of all previous visits to or residence in the United States. (if never, write "never") Give type of visa status, if known. Give DHS "A" number if any."

For just about 5yrs now I have been driving over the border to visit my fiance at least Once a month but I have never stayed past 7 days. They didn't stamp my visa so I can't give exact dates. Do I have to list every time I crossed the border? I have been to the States on vacation 8+ years ago but can't remember the exact dates can I put unknown?

Questions 19 and 24 "Mother and Father's Family Name" Both my parents were raised with different names than their birth names. I knew my father went by a different name because he changed his last name to his birth name, but I didn't know my mom had a differnt name on her birth certificate and when I filled out the I-129F and sent it in I put the name she grew up with, but when I applied for my birth certificate it came back that I didn't have the right name. It turned out my mom was given her mothers maiden name at birth but was raised wth her fathers name. Is that going to be a problem? Can I put both names my parents used and indicate which one is on the birth certificate? Or do I need to get a letter from them explaining why I put the wrong name on the I-129F?

i just filled out the form and send it in. so i feel u at some questions i was all like huh what???

with the living thing. i moved a lot as well lol and i put everything in it. if u need more space u can use a extra page to fill it out. i would put it what u know.

the most important thing when u fill out these forms is to be HONEST. dont give them any reason to deny ur visa. so be honest and fill it out as much u can.

Biometric appt: 05-18-2012

AOS transfer to CSC: 05-24-2012

EAD/AD approved&send: 06-22-2012

EAD/AD arrived: 06-28-2012
