Hi everyone, pls i will be posting my AOS package soon and will like to know if the vaccination supplement that was required is the one i have from my country already or will i have to go somewhere to get another one? Also, since am married now will i use my husband family name or my maiden name to fill the forms cos i will like to change my name to his family name. thanks for every opinion cos they counts.

You can fill out the AOS forms in your husbands name if you like. As for the vaccination records, some people have gotten by just sending the DS-3205 while some have it transcribed by a civil surgeon onto a i-693. It's up to you.

My Journey:

We met through a study-abroad program in Shanghai, China in August of 2009

We got engaged March of 2010

I received my K1 VISA in 6 months (June-December 2010)

We were married 04/02/2011
I received my conditional 2-year greencard (AOS) in 2.5 months with no interview (April-June 2011)

Our son was born 02/03/2013

I received my masters degree in Speech-Language Pathology 04/17/2013

I received my 10-year greencard (ROC) in 3 months with no interview (March-June 2013)

My husband returned from deployment 06/20/2013

My naturalization journey took 4 months (April-August 2014)

I became a US citizen on 08/01/2014

Received passport in 3 weeks (regular processing)

yes you can use your married name no problem because on the form it ask for your married name and maiden name...... To be on the safe side In my opinion I would get the vaccines transcribed....Good luck good to see you around again :)

I agree with Patient, get the vaccines transcribed. As for the name, I think you should use your married name. One less headache to deal with during the name change process.

We were approved just today and used only the DS-2035. No RFE's and were approved in less then two months.

You can stand me at the gates of hell, but I wont back down.
