Hello,

While searching the US Embassy Canberra, website I noticed this requirement under

'Visa Instructions for Fiancee Non-Immigrant Visa Applications':

"4.When you have gathered all documentation, complete and mail the DSL-1076 form to the U.S. Consulate General, Sydney. That office will contact you with your visa interview time. Please do not fax forms DS-230 Part I and DSL-1076, the originals must be mailed."

Other official web sites don't mention this form. Could someone enlighten me please?

Sounds like Department of state form that is consulate specific.

Other names for this are DS-2001, OF-169, is a list of required documents needed for interview, and that you have obtained then and are ready to interview.

Some forms are not available for download.

The form he is talking about Dan is just the instruction form on the packet 3 it is a checklist so to speak and you sign it.

Divorced !st November 2012.
