"Quit Claim" for minor child

Hi everyone. We are just starting the process to prepare our K1 application. This forum has been provided us with so many helpful tips and clarifications. However, one topic I have not seen reference to, is the follow document listed on the HCMC consulate web page.

"9.An original, certified and signed Quit Claim for all minor children (age under 18) giving their other parent’s permission for them to accompany the applicant to the U.S."

We will need to get this signed by my fiance's ex-husband. We want to make sure it is correct, but I have not been able to find any examples of this form or at least, example wording.

Does anyone have experience with this and could point us to a sample?

Thanks to all the various posters who take their time to help those of use just starting on this "journey".

2012-01-12 - Travel to US. Chicago POE

2012-04-05 - Biometrics

2012-05-02 - EAD for spouse and stepson

2012-05-03 - Apply fo SSN(stepson)

2012-05-29 - Interview

2012-05-29 - I-485 Approved

2012-06-06 - Green Cards received

There's no standard format. What is required is a statement from the non-immigrating parent stating that they give their permission for their child or children to immigrate permanently to the United States. The statement must signed, dated, notarized, and translated. You'll also need a photocopy of the non-immigrating parent's national ID card or passport.

05/01/2013 - Removal of Conditions - APPROVED!
