maiden name error

I mistakenly put my married name in the spot on the G325 my husband had to sign...are you able to white out..draw a line through and initial....whatever to change it to my maiden name or do I have to have him send another form? I could kick myself!

The proper way to correct an error is to draw a single line through the error and, provided there is room write the correct entry. If there is no room do as mentioned above and write see attachment. Also, it is standard practice to initial the change. NEVER use white out.

There is room for the change. I didnt even notice, my husband said to me last night you realize you put McGonigle where it says maiden name is that right? Which I responded with a big F$&K! ha! That was the only thing I was waiting to get back before I sent the packet off so I was cursing myself!
