Form DS-156 "Bad Year" error

I am filling out my fiancee's DS-156 form. I checked over many times to see if the information is correct. However, when I go to click the continue button, a webpage message pops up and says "Bad year 'JAN2'. Must be four digits". Then it takes me to question number 4 "issuance date" and nothing else happens. It just highlights the day box. Her passport issuance date happens to be in january. Whenever i change the month out of curiosity, (December for example) the webpage message changes and says "Bad Year 'DEC2'. Must be four digits". I don't know what is the problem. If anyone knows how to fix this or has gotten the same problem and fixed it please let me know. I am using the correct website also at https://evisaforms.state.gov/ds156.asp

well i would guess its because it needs to be in the format 01/02 for 2nd jan....

All the dates on that online form shows dd month YYYY Dates are 01,02...31 months are a roll down selection years are 4 digits, 1980, 1981 ... 2010 etc..

Yes. The specific Issue date is 03 January 2011. And that's the way I entered it. But for some reason I keep getting the same error

Ok problem solved. This website is not compatible with Internet Explorer 9. I used a computer with IE 8 and it worked just fine. Hope this helps anyone else with this problem in the future. Thanks for your input
