hi my fiance got the K-1 visa and now in the US.

I am trying to apply for I-485.

My question is do I need medical record ( vaccination supplement)?

I take it you are married? If your medical is no older than 12 months then you are fine and if your vaccinations are complete then you should have a ds-3025 a vaccination supplement. It is advised though to see a civil surgeon to have your vaccinations only transcribed over on to a I-693 part 1.2 and 5 vaccination only. You can send your supplement off with I-485 if all vaccinations are complete and possibly not get an RFE. We used the I-693 to be on the safe side.

Yes, you will need any vaccination record you brought with you transferred to an I-693, however be aware many doctors like hitting you for a FULL medical, shop around, you only need the vaccination part of the form completed if your medical for visa happened less that 1 year prior to fileing adjustment of status.

Google USCIS Designated Civil Surgeons. Call some and get prices. LOTS will try and make you/her do a full medical and rip you guys off. Your fiance only needs the vaccinations transfered from the DS 3025 to the I-693.

Keep original DS3025.
